Dr. Rifat has again generously offered to provide sports physicals for Notre Dame students. Physicals will be offered for the 2017-18 school year on Friday, June 2nd beginning at 12:00.
The cost is $25 per student. In order to participate in athletics next school year all athletes must have a physical on file with the Athletic Department dated after April 15, 2017.
Space is limited. Please register by calling the athletic office at 248-373-9647 or emailing Ms. Leseman at mleseman@ndpma.org.
All students must bring a completed sports physical consent form (see link below) with them on June 2nd.
